Welcome
We invite and encourage you to become a member of our parish family at St. Edward Church in Herminie, Pennsylvania.
We are an active faith community of nearly 700 registered households, and we strive to deepen the faith and to broaden the vision of church for all parishioners. Since Christ, present in the celebration of the word and sacrament, is the source and summit of our faith, our celebration of the liturgy is the life blood of our parish.

When completing the form please follow the instructions listed below:
- All dates require a month, day and year. If the date is unknown, but the sacrament was received, just circle yes.
- Enter the membership in church organizations, ministries (lector, altar server, choir, etc.) or any volunteer work being done, or talents/hobbies that you might have.
- Please use the back of this form for additional space if needed, comments or remarks. If others are living in the home, besides your children, please explain on reverse side.
- Adult children, who are not in college, should fill out their own registration form and receive their own parish offertory envelopes.
Please return this parish registration form, completed in full, by mail or in the offertory basket on during a weekend Mass.
